Patients are generally advised to start Lioresal at a lower dosage and gradually increase the medication until the beneficial effect is achieved. Starting at 5 mg taken three times a day on the first three days, most patients are taking 20 mg by day twelve. In man, a single 10 mg oral dose of baclofen is rapidly and almost completely absorbed whereas absorption of 20 mg and 40 mg doses is less complete.
The concomitant administration of baclofen and tricyclic antidepressants may potentiate the pharmacological effects of baclofen, resulting in pronounced muscular hypotonia. The concurrent use of MAO inhibitors and baclofen may result in increased CNS depressant effects; therefore, caution is advised and the dosage of one or both agents should be adjusted accordingly. Isolated cases of increased blood glucose concentrations have reported with baclofen; dosage adjustments of antidiabetic agents may therefore be necessary with combined baclofen treatment. As a general rule, nursing should not be undertaken while a patient is on a drug.
Psychotic disorders, schizophrenia, depressive or manic disorders, confusional states or Parkinson's disease may be exacerbated by treatment with Lioresal. Lioresal may also exacerbate epileptic manifestations but can be employed provided appropriate supervision and adequate anticonvulsive therapy are maintained. Lioresal should be used with extreme care in patients already receiving antihypertensive therapy. Lioresal should be used with caution in patients suffering from cerebrovascular accidents or from respiratory, hepatic or renal impairment. Signs of overdose have been observed in patients with renal impairment taking oral Lioresal at doses of more than 5mg per day.
